Last year, the US Census Bureau reported that 43.6 million Americans live in poverty, including 3.4 million Americans over the age of 65. In San Francisco, amid damaging budget cuts to healthcare and high costs of living, the Transitional Care Network responds to the unmet needs of the commonly voiceless, but growing, diverse population of urban poor and elderly residents who are aging in place, yet increasingly isolated. The images explore this population, their environment and "aging in place".
